Does not include uniform, practice jersey or helmet/gear bag.

- I hereby agree that my child and I will adhere to the Skokie Indians Little League Parent and Player Codes of Conduct at http://skokieindians.org/parent-code-of-conduct/ and http://skokieindians.org/player-code-of-conduct/ I hereby give my approval for this child to participate in any and all activities of Skokie Indians Little League Baseball and its leagues, and agree that my child and I will follow all league rules of play and conduct. I understand that baseball is a sport which carries risks including collisions, tripping and being hit by a ball or bat, and that protective equipment does not prevent all injuries to players, and do hereby waive, release, indemnify, and agree to hold harmless Skokie Indians Little League Baseball and its leagues, Little League Baseball, Inc., the Skokie Park District, the organizers, sponsors, supervisors, participants, managers, coaches and persons transporting my child to and from activities, from any claim arising out of an injury whether the result of negligence or any other cause. I also understand that once uniforms are ordered only 50% of the registration fee will be refunded. After the first game of the season, there will be no refunds. Returned check fee is $25.00. I understand that photos/videos are periodically taken of people participating in the SILL and I agree that any photograph or video taken by members of SILL of me and/or my minor child/ward while participating in SILL activities and events may be used by SILL for promotional purposes including the newsletter, electronic media, brochures, flyers, and other publications without additional prior notice, permission or compensation to the participant.
